Jobing Description

Provide telephone outreach, basic health education and health coaching to identified high-risk clients with metabolic syndrome. Assist clients in registering for wellness classes and groups, and provide follow up services. Provide exceptional customer service and documentation support for Jefferson Center clients and co-workers.

  • Provide health consultation and education to clients enrolled in Jefferson Center’s Healthcare Management Program. Meet with clients in coordination with prescriber appointment for development of initial brief health plan, and monthly follow up to address progress, both face to face and telephonically.    
  • Provide active follow up, support and outreach to persons with serious mental illness and their families, and other low-income community members with mental health/wellness needs. This includes telephone outreach, basic health education and coaching to individuals who have received health screenings from Wellness staff or PCPs and are not in the Healthcare Management Program.
  • Assist participants to achieve individualized health plan goals through      motivational techniques and coaching.
  • Assist clients to enroll in relevant Wellness classes, and to access community resources that will promote achievement of their health goals, including healthy eating and active living.
  • Assist Wellness team to track and achieve milestones/outputs and longer-term participant outcomes for the Colorado Health Foundation grant in the areas of healthy living.
  • Liaison between physical and mental healthcare providers, and between Jefferson Center and other agencies or providers. This should include but is not limited to assisting consumers in navigating each system, assisting clients with referrals to each system, and facilitating communication between providers.
  • Provide case management services to assist clients in obtaining both medical and psychiatric care including, but not limited to, service planning, monitoring illness, assistance making and keeping follow-up provider appointments, referrals to appropriate agencies for benefits and entitlements, advocacy, and crisis management for consumers.
  • Make appropriate referrals to, and act as liaison with, community agencies, service providers, and natural support systems such as neighborhood networks, churches, police and probation officials, social services, etc.
  • Work as a part of the Wellness team to promote client participation in diet, exercise, smoking/tobacco use cessation and other health change classes and education, and follow up on individual health plan goals and objectives.
  • Ongoing monitoring of assigned caseload and align clients’ improvements in their illness with treatment goals; assess symptoms in order to prevent relapse.
  • Participate in team meetings, clinical staffings, and clinical consultation as needed at Jefferson Center.
  • Attend mandatory trainings as required by Jefferson Center.
  • Participate in supervision as agreed upon with supervisor.
  • Complete all documentation and other clinical performance standards of Jefferson Center.
  • Exhibit enthusiasm, courtesy, adaptability, flexibility, and spirit of cooperation in the work environment. Maintain appropriate professional standards.
  • Maintain effective interpersonal relations.
  • Effectively respond to patient/client needs and problems, initiate and maintain positive interactions, timely response to phone calls, pages, email and other requests.
  • Work cooperatively with other community agencies, as appropriate, and in agreement with supervisor.

Skills / Requirements

Minimum of a BA, in psychology, social work or related field, or Medical Assistant with Certification/Diploma. Experience in both physical (medical) and behavioral health care, especially with SPMI populations. Experience working in integrated care in community mental health or primary care desired. Knowledge and work experience in motivational interviewing, and life or health/wellness or chronic disease coaching, certification preferred. Bilingual preferred in English/Spanish. Excellent skills in coordination of services are essential. Individuals who are consumers of mental health services who meet the qualifications are encouraged to apply.
