Jobing Description

To service the pilot project with CCHA and FBHP under the State RCCO contract for care coordination. To provide integrated healthcare coordination and management, advocacy, guidance and support to attributed CCHA patients residing in Jefferson County with primary behavioral health diagnoses and to their families. 

  • Coordinates the delivery of services to patients, including collaborating with clinical and other staff to address patients’ needs.
  • Assesses barriers to health and determines appropriate ways to address those barriers.
  • Evaluates patient’s condition and develops care plans to improve overall health situation.
  • Offers the patient guidance and education related to her/his specific needs.
  • Connects patients to resources within Jefferson Center and the community to address identified health concerns.
  • Prepares for and participates in case review sessions.
  • Provides timely documentation and required reports for all care coordination activities.
  • Provides input into the service delivery systems and care coordination protocols and procedures for Accountable Care based on evidence-based models (e.g., Eric Coleman model).
  • Attends all staff meetings and all other meetings as assigned.
  • Demonstrate appropriate problem-solving skills to promote sustainable self-care for patients.
  • Complete all mandatory training as required by Jefferson Center.
  • Participate actively in supervision as required.
  • Exhibit enthusiasm, courtesy, adaptability, flexibility, and spirit of cooperation in the work environment. Maintain all appropriate professional standards.
  • Maintain effective interpersonal relations.
  • Effectively respond to patient needs and problems, initiate and maintain positive interactions, timely response to phone calls, pages, email and other requests.
  • Work cooperatively with other community agencies, as appropriate, and in agreement with supervisors.

Skills / Requirements

  • Undergraduate Bachelors degree in Health or Human Services or related field.
  • Prior experience working in social work, behavioral health or community health is strongly preferred.
  • Must be professional, attentive to detail, able to work effectively and collaboratively on interdisciplinary healthcare teams, and have excellent interpersonal skills with patients, colleagues and management.
  • Must be able to work independently in a manner that ensures accuracy and efficiency.
  • Must be able to discern information from others in a variety of formats and communicate information to others in a manner that helps them understand instruction.
  • Must demonstrate strong professional boundaries and ability to respond effectively, sensitively, and with empathy to the needs and concerns of a diverse patient population.
  • Keyboarding skills and facility with computer software including but not limited to the internet and MS Office Products are required.
  • Must learn and document in the Jefferson Center electronic health record.
  • Must meet criteria for and complete credentialing process as needed at Jefferson Center.
  • Must be able to protect the privacy and confidentiality of patients, employees and business information.
  • Bilingual helpful but not required.
